• Integration with Florida’s Health Information Exchange to immediately capture discharged patient from in-patient settings for continuity of care.
• Integration with Athena to allow synchronization of patient records and the uploading of patient forms, assessments, screenings, and insurance information into Athena
• Patient engagement functions including registration, scheduling, telehealth, secure messaging and care coordination to actively engage and manage patient outcomes, crucial to succeeding as an ACO.
• Seamless integration of RPM into the patient engagement platform allowing care managers to actively engage patients and monitor RPM data with real-time thresholds and analytics.
